THIS NOTICE DESCRIBES HOW YOUR MEDICAL INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Uses and Disclosures of PHI

We may use your Protected Health Information (PHI) for treatment, payment, and healthcare operations, including coordination of care, billing, insurance verification, and quality improvement.

2. Business Associates

We may share PHI with third-party vendors under strict agreements requiring HIPAA compliance.

3. Permitted Disclosures

4. Reproductive Health Privacy (2024 Rule)

We will not disclose reproductive health information for investigations or legal action without proper legal requirements.

6. Your Rights

7. Breach Notification
