- 1 comprehensive exam and 1 emergency exam (used at any time during the year)
- 2 regular prophy cleanings (non-periodontal)
- 3 periodontal maintenance cleanings at 30% off
- 2 oral cancer screenings
- 2 fluoride tooth desensitizing treatments
- 4 bitewing x-rays
- Any individual x-rays needed throughout the year
- 10% off minor services such as, but not limited to: dental sealants, fillings, or extractions (Check with office for further details)
- 15% off major services such as but not limited to implants, crowns, and veneers. (Check with office for further details)

Program Guidelines
- Patients are responsible for paying their portion the day of services rendered.
- There will be a $50 reinstatement fee if your plan lapses
- No refunds of membership payment
- NON-REFUNDABLE
Program Exclusions & Limits
Cannot be used in conjunction with another dental plan, dental insurance, or financing such as the following:
- *For treatment which, in the sole opinion of our doctors, lies outside the realm of their capacity.
- *For referrals to specialists.
- *For hospitalizations or hospital charges of any kind.
- *For cost of dental care which are covered under automobile medical.
- *For services of injuries covered under automobile medical.
