Cancellations & Missed Appointments

A minimum of 48 business hours’ notice (Mon–Thurs, 8 AM–5 PM CST) is required. Late cancellations or no-shows may incur a $50 fee.

Payment Terms

Payment is due in full at time of service. We accept cash, check, major credit cards, HSA, insurance, and financing options (Sunbit, CareCredit, Cherry).

Insurance Responsibility

Insurance is a contract between you and your provider. Any remaining balance after insurance processing is your responsibility.

Outstanding Balances & Finance Charges

Balances unpaid after 60 days may incur a finance charge of 1.5% per month (18% annually). Charges are applied monthly to the remaining balance until paid in full.

Additional Fees

Returned payments incur a $25 NSF fee. Unpaid balances may be referred to a collections agency, and you may be responsible for collection costs where permitted by law.

Communication

We may contact you via phone, text, or email regarding appointments and billing. These methods may not be fully secure.

Wisconsin Financial Policy

The parent or guardian accompanying a minor is responsible for payment. Divorce decrees do not obligate this office to divide billing.
